I. GRADUATING HIGH SCHOOL STUDENTS

Only candidates for high school graduation and graduates of the current school year are immediately qualified to apply for admission and take the Lyceum's College Entrance Test (CET).

Applicants must submit the following requirements:

• Two (2) pcs. 2"x2" recent, identical pictures (colored, any background)
• Photocopy of the 4th Yr. Report Card (indicating latest grading period)
• Certification of Candidacy for Graduation and of Good Moral Character
• Photocopy of Birh Certificate
• Testing fee of Php400.00

Note: Foreigners graduating from local high schools should present their birth certificate and applicable alien papers

 ADDITIONAL REQUIREMENTS FOR BS NURSING

• Photocopy of the 4th Yr. Report Card (indicating latest grading period)
• GPA of at least 80%
• At least 5ft. in height, desirable body weight
• Two(2) pcs. long brown envelope

 II. OTHERS

Applicants classified as any of the following, must present their requirements to the Admissions Information Center before they are referred to the Registrar's Office for an admission clearance:

1. High school graduates of previous school years
   • Original 4th Yr. Report Card & Certification of Good Moral Character
   • Certification from the HS registrar that the applicant's Form 137-A is still with the former and that it has not been released yet to any college, university or abroad
   • Two (2) pcs. 2" x 2" recent identical pictures (colored, any background)
   • Testing fee of Php400.00
   
   
2. Transferees (Undergraduate students from other school,
   Graduates of any 2, 4, or 5 yr. Course)
   • True Copy of Grades/ Temporary Transcript/ Certification of Grades/ Transcript of Records (if available)
   • Certification of Good Moral Character
   • Honorable Dismissal/ Transfer Credentials (if available)
   • Two (2) pcs. 2" x 2" recent identical pictures (colored, any background)
   • Testing fee of Php400.00
   
   
3. Graduates of Non Formal Education (NFE)
   • Original NFE result report
   • DECS Certification of eligibility for college admission
   • Two (2) pcs. - 2" x 2" recent identical pictures (colored, any background)
   • Photocopy of Birth Certificate
   • Testing fee of Php400.00
   
   
4. Philippine Educational Placement Test (PEPT) holders
   • Original PEPT result report
   • DECS Certification of eligibility for college admission
   • Two (2) pcs. - 2" x 2" recent identical pictures (colored, any background)
   • Photocopy of Birth Certificate
   • Testing fee of Php400.00
   
   
5. Philippine Validating Test (PVT) holders
   • Original PVT result report
   • DECS Certification of eligibility for college admission
   • Two (2) pcs. - 2" x 2" recent identical pictures (colored, any background)
   • Photocopy of Birth Certificate
   • Testing fee of Php400.00
   
   
6. Filipinos who studied / graduated abroad
   • Authenticated school records from the Philippine Embassy in the relevant countries of schooling
   • Birth Certificate
   • Validated Passport

 III. FOREIGN STUDENTS AND RESIDENT ALIENS

• Foreign students and resident aliens will undergo an initial interview at the Registrar's office regarding their particular requirements for entrance examinations.
• Testing fee of $150.00

Test registration is on a first-come, first-served basis.
Schedule will be given upon application.
